Gurgaon: A 27-year-old woman with a rare condition, diagnosed with a left unicornuate uterus accompanied by adenomyosis in the non-communicating right horn, underwent a successful five-hour surgery led by Dr. Aruna Kalra, director of the obstetrics and gynecology department at CK Birla Hospital in Sector 50. Following the procedure, she was discharged home within a day. Dr. Kalra highlighted that a unicornuate uterus, characterized by having only one fallopian tube, combined with adenomyosis in the non-communicating right horn, constituted a rare Mullerian malformation. The patient, residing in Sector 50, presented in February with complaints of pelvic pressure, persistent pain localized to the right side of her lower abdomen and pelvis, and difficulties conceiving after three years of marriage. Clinical examinations revealed a large, firm lump displaced to the right side of the lower abdomen, prompting further investigation. Describing it as an "exceptional rarity case" of a functioning non-communicating rudimentary horn combined with adenomyosis, Dr. Kalra emphasized the surgical procedure's meticulousness, involving careful dissection and extraction while preserving adjacent structures such as the intestines and urinary bladder. Through meticulous dissection, the medical team successfully excised the adenomyoma, safeguarding the integrity of the right ureter. After being admitted on February 15 for the intricate surgery, the woman returned home from the hospital within a day. Reflecting on her experience, she expressed, "Before the surgery, I endured constant pain and pressure in my lower abdomen and pelvis, which greatly impacted my physical and emotional health. I had nearly lost hope of conceiving. However, following the surgery, the relief I feel is indescribable. It has truly given me a fresh start." "After conducting a comprehensive assessment, including ultrasound and MRI scans, initial suspicions pointed towards a fibroid. However, further investigation through detailed diagnostic hysteroscopy and robotic myomectomy (adenomyoma removal) with chromopertubation unveiled an unexpected and exceedingly rare discovery — adenomyoma in the non-communicating horn," explained Kalra. She added that the patient is now poised to embark on her fertility journey. The successful treatment encompassed a thorough robotic-assisted excision of the entire horn, leading to a significant improvement in the patient's condition.
